If you are still interested in egg donation after reviewing the above information, contact our Donor Coordinator at (616) 988-2229, ext. 131. She will provide the paperwork necessary for you to begin the process, including a medical record release so that we can review your medical records and completing a donor history questionnaire. She will review these records--if you qualify and we can find a recipient match for you, you will be contacted to continue the process of egg donation.
